Reproductive, Maternal, Neonatal and Child & Adolescent Health Programme

 

Introduction

The Reproductive and Child Health (RCH) Programme was launched in October 1997. The main aim of the programme is to reduce infant, child and maternal mortality rates.

To ensure "Continuum of Care" for reproductive, maternal, newborn and child health (RMNCH) includes integrated service delivery for mothers and children from pre- pregnancy to delivery, the immediate postnatal period, and childhood.

Reproductive Health Interventions

Various temporary and permanent family planning options are provided free of cost to the eligible beneficiaries under the programme.

 

Maternal Health Interventions

Under Maternal Health the key components are as under:

·        Surakshit Matritva Aashwasan (SUMAN)

·        Pradhan Mantri Surakshit Matritva Abhiyaan (PMSMA)

·        Janani Shishu Suraksha Karyakram (JSSK)

·        Janani Suraksha Yojana (JSY)

·        LaQshya

·        Comprehensive Abortion Care (CAC)

Surakshit Matritva Aashwasan (SUMAN)

Assured, dignified, respectful and quality healthcare at no cost and zero tolerance for denial of services for every woman and newborn visiting the public health facility to end all preventable maternal and newborn deaths and morbidities and provide a positive birthing experience.

 

Pradhan Mantri Surakshit Matriva Abhiyan (PMSMA)

The PMSMA has been implemented in A&N Islands on 9th September,2016.   On 9th day of every month all essential maternal health services are provided at all public health facilities. The PMSMA is aimed to reach out all pregnant women who are in 2nd & 3rd trimester of pregnancy.

It is to ensure that all pregnant women complete their scheduled Antenatal Care Visit and all essential investigation.

The detected High Risk Pregnant Women are referred to higher centre for further management by the Gynecologist.

3692 pregnant women were examined during the year 2023-24.


Janani Shishu Suraksha Karyakaram (JSSK)

JSSK is an initiative to provide completely free and cashless services to pregnant women including normal deliveries and caesarean operations and sick new born (up to 1 year after birth) in Government health institutions in both rural & urban areas.

The following are the Free Entitlements for pregnant women:

·        Free and cashless delivery

·        Free C-Section

·        Free drugs and consumables

·        Free diagnostics

·        Free diet during stay in the health institutions

·        Free provision of blood

·        Free transport from home to health institutions

·        Free transport between facilities in case of referral

·        Free drop back from Institutions to home after 48hrs stay

 

The following are the Free Entitlements for Sick        newborns till 1 year after birth:

·        Free treatment

·        Free drugs and consumables 

·        Free diagnostics

·        Free provision of blood Exemption from user charges

·        Free Transport from Home to Health Institutions

·        Free Transport between facilities in case of referral

·        Free drop Back from Institutions to home

 

 Janani Suraksha Yojana (JSY)

 

Janani Suraksha Yojana (JSY) is a safe motherhood intervention under the National Health Mission (NHM). To reduce maternal and infant mortality by promoting institutional delivery among the poor pregnant women. In this scheme the Beneficiaries belongings to BPL/ST. JSY integrates cash assistance with delivery and post-delivery care.

 

LaQshya

 

After launch of the National Health Mission (NHM), there has been substantial increase in the number of institutional deliveries. However, this increase in the numbers has not resulted into commensurate improvements in the key maternal and new-born health.

The programme is envisaged to reduce preventable maternal and newborn mortality, morbidity and stillbirths associated with the care around delivery in Labour room and Maternity OT and ensure respectful maternity care.

Objectives:

To reduce maternal and newborn mortality & morbidity due to APH, PPH, retained placenta, preterm, preeclampsia & eclampsia, obstructed labour, puerperal sepsis, newborn asphyxia, and sepsis, etc.

To improve Quality of care during the delivery and immediate post-partum care, stabilization of complications and ensure timely referrals, and enable an effective two-way follow-up system.

To enhance satisfaction of beneficiaries visiting the health facilities and provide Respectful Maternity Care (RMC) to all pregnant women attending the public health facility.

Scope:

The LAQSHAY programme is proposed in the following health facilities of the Andaman & Nicobar Islands namely:

G.B.Pant Hospital, Sri Vijaya Puram, RPH Mayabunder, BJRH Car Nicobar, CHC Diglipur, CHC Rangat and DH Garacharma.

 

Comprehensive Abortion Care (CAC)

Comprehensive abortion care (CAC) is an integral component of maternal health interventions as part of the National Health Mission. Abortion is a cross-cutting issue requiring interface with not just girls and women but across all age groups.

 

Steps Taken Under Maternal Health

·        ANC and PNC services have been provided in all the institutions of A&N Islands.

·        Periodic capacity building of Medical Officers, ANMs and other Paramedical Staff.

 

New Born And Child Health Interventions

 

 

Facility Based Newborn Care (FBNC)

 

·        Sick New Born Care Unit (SNCU) – 1 at GBPH

·        New Born Stabilization Unit (NBSU) – 6 at DHs and CHCs

·        New Born Care Corners (NBCC) – 20 at PHCs level

 

Sick Newborn Care Unit

The Sick New Born Care Unit is established with the view to provide supportive aid to the premature and sick newborns.

 

Home Based Newborn Care (HBNC) & Home-Based Young Child Care (HBYC)

Under HBNC and HBYC Programmes, Home visits are performed by ASHAs to improve Child rearing practices and to identified sick new born and young Children in the community.

 

Universal Immunization Programmes

UIP is implemented to provide the vaccination to children against life threatening diseases.

 

Infant And Young Child Feeding (IYCF)

A global strategy for Infant and Young Child Feeding (IYCF) was issued jointly by the World     Health Organization (WHO) and the United Nations Children's Fund (UNICEF) in 2002, to reverse the disturbing trends in infant and young child feeding practices.

Series of activities to commensurate the IYCF activities includes MAA Programme, POSHAN MAA. Under ‘MAA’ Programme imperative that efforts are intensified to improve optimal breastfeeding practices

Early initiation of breastfeeding within one-hour, Exclusive breastfeeding for the first six months, and continued breastfeeding for at least two years.

 

Rasthriya Bal Swasthiya Karyakram (RBSK)

Rashtriya Bal Swasthya Karyakram (RBSK) is an important initiative aiming at early identification and early intervention for children from birth and developmental related defects in the age group of 0-18 years. Presently 06 dedicated Mobile Health Team comprising of 10 Ayush Medical Officers, 4 staff nurse, 4 pharmacists are engaged in the screening activities in the three districts of the islands.

 

Adolescent Health-Rastriya Kishor Swasthiya Karyakram(RKSK)

The Rashtriya Kishor Swasthya Karyakram (National Adolescent Health Programme), will comprehensively address the health needs of the adolescents. It introduces community-based interventions through peer educators,

Objectives:

To improve the adolescent health (AH) (in the age group of 10 to 19 years) to improve nutrition by reducing the prevalence of malnutrition and iron deficiency anaemia.To improve knowledge, attitude and behaviour in relation to sex and reproductive health (SRH).

To Prevent substance misuse by increasing adolescent’s awareness on the adverse effect of substance misuse.

To promote behavioural change in adolescent to prevent non communicable diseases.

Community based interventions:

·        Peer Education (PE)

·        Quarterly Adolescent Health Day (AHD)

·        Weekly Iron and Folic Acid Supplementation Programme (WIFS)

·        Menstrual Hygiene Scheme (MHS)

Facility based interventions:

Strengthening of Adolescent Friendly Health Clinics (AFHC) within Health & Family Welfare - FP, MH (incl VHND), RBSK, National Tobacco Control Programme, National Mental Health Programme, NCDs and IEC with other departments/schemes – Social Welfare (ICDS, SABLA), Youth Affairs and Sports etc.

School Health & Wellness Programme:

It is a part of the Health and Wellness component of the Ayushman Bharat Programme. School Health & Wellness Programme (launched in Feb 2020) is being implemented in government and government aided schools in districts (including aspirational districts). Two teachers, preferably one male and one female, in every school, designated as “Health and Wellness Ambassadors” shall be trained to transact with school children, health promotion and disease prevention information on 11 thematic areas in the form of interesting joyful interactive activities for one hour every week.

Weekly Iron Folic Supplementation Programme (WIFS):

During adolescence, iron deficiency anaemia can result in impaired physical growth, poor cognitive development, reduced physical fitness and work performance and lower concentration on daily tasks Anemic adolescent girls have a higher risk of preterm delivery and having babies with low-birth-weight Weekly Iron and Folic Acid Supplementation (WIFS) Programme has been launched to meet the challenge of high prevalence and incidence of anaemia amongst adolescent girls and boys.

Activities

·        Two nodal teachers from each school have been trained in WIFS

·        Supervised distribution of IFA blue tablet by nodal teacher on every Monday after MDM.

·        IFA tablets have been procured and distributed through area ANM

·        IEC materials on hand washing have been distributed to all the school

·        IFA syrup has been procured and distributed to AWC

·        Supervised administration of 1 ml IFA syrup twice a week by AWW

Training

·        29 ANM’s trained for Adolescent Friendly Health Services.

·        03 days training on Home Based Care for Young Child HBYC was conducted altogether 30 ANMs, CHOs and PHNs were trained.

·        NSSK

·        71 Peer Educators and 9 ASHA worker trained under Peer Educator Programme

·        111 Health and Wellness Ambassadors were trained under Ayushman Bharat School Health and Wellness Programme.

·        70 Principal oriented on Ayushman Bharat School Health and Wellness Programme.

Activities

·        Half day orientation for ASHAs under MAA Programme during Breastfeeding Week. Altogether 65 ASHAs were oriented. 

·        During World Pneumonia Day under SAANS Programme and National Newborn Week 2023, 77 Nos. CHO, ANMs, AANMs, ASHAs and Anganwadi Workers were oriented on identification and management of pneumonia and newborn care.

·        50 health facilities identified and notified under SUMAN.

·        973 Health & Wellness Messengers selected in South Andaman, North & Middle Andaman and Nicobar District.

·        RCH Camp conducted at Mannarghat and Wandoor
